Self-Referral and Physician Referral:

Depending on your insurance plan, you may be able to self-refer, meaning you can schedule an appointment without needing a physician’s referral. However, certain plans require a referral from a physician before you can book an appointment. If your plan requires a physician referral, we kindly request that you arrange this before scheduling your appointment. When you visit any of our clinics, we recommend that you dress comfortably and consider bringing a light sweater or shawl for warmth. We also encourage you to bring a family member or friend along for support. Once your appointment is confirmed, start preparing the items you’ll need, which include:

• List of current medications and dosages
• Pathology slides
• Scan and X-ray films
• Copy of your insurance card (front and back)
• A book, tablet, or laptop for entertainment while you wait (free WiFi is available)

In some instances, you may need to assist us in obtaining records from your current or referring physician. Much of this information can be faxed to our office ahead of your initial visit. We will guide you on what we need and why, which may include:
