Self-Referral and Physician Referral:

Navigating insurance policies can be complex, especially regarding referral requirements. Some plans permit self-referrals, allowing patients to book appointments directly, while others require a referral from a physician. If your plan necessitates a physician referral, we kindly request that you secure it before scheduling your appointment. Co-payments:

It’s important to note that co-payments are required at the time of service. If your specialist co-payment amount isn’t mentioned on your insurance card, we advise contacting Member Services or your insurance company’s Customer Service (using the contact number on your card) to ascertain the exact amount.

Insurance Card(s):

We ask that you provide your insurance details to the coordinator when scheduling your appointment and bring your insurance card(s) to your appointment. This allows our Financial Counselor to make necessary copies during your visit.
